Program Description

The Community Mental Health Promotion Program (CMHPP) is part of the Mental Health and Street Outreach Service (MHSO). The Community Mental Health Promotion Program (CMHPP) provides case management service for individuals living with a diagnosed serious and persistent mental illness and are in stable housing. The MHSO Service also provides engagement services where street outreach workers connect with individuals who are experiencing homelessness. The case management team of the CMHPP involves Public Health Nurses working with the client in the community where individual lives to: 1) promote the achievement of optimal health and well-being; 2) to increase independence and participation in community life. The model of service is client directed, recovery and harm reduction focused.

Access Information

Before accessing this program the client must complete the intake process through IntAc. IntAc is the central point for referrals to 4 mental health case management organizations. The Admission Team of IntAc decides which of the 4 programs would best be able to meet the client's defined needs.
