Program Description

Cota Health's Mental Health and Justice Supportive Housing Program provides rent subsidized housing and supports to people with mental illness who are homeless and are involved with, or are at significant risk of involvement with, the justice system. Tenants of this program occupy individual apartment units in locations throughout the communities of Scarborough and North York. Housing support is provided to tenants through scheduled individual visits and coordinated group activities. The staff work collaboratively with tenants, their families, landlords and other service providers, as appropriate to assist tenants to: develop an individual safety and housing support plan; maintain their housing stability and a healthy housing environment; acquire and/or maintain the skills necessary for daily living; participate in health promotion and community development activities such as tenant meetings, structured educational events and social activities; resolve situational problems/disputes; link to other desired community services/supports, as needed; achieve/maintain optimum health and wellness; and pursue personal recovery.

Access Information

Referrals go through The Access Point. This program is available to men and women who are 16 years of age or older and are: living with serious mental illness; actively involved with the justice system or at significant risk of becoming involved with the justice system; experiencing homelessness, or are at significant risk of becoming homeless; assessed as being able to be safely supported by the program, given the limitations of resources; applicants must also have some available means of financial support such as the Ontario Disability Support Program (ODSP) or Ontario Works (OW).
