Program Description

The Housing Outreach Program exists to help chronically homeless individuals living with a severe and persistent mental illness find housing in the community and work towards addressing issues that may have contributed to their homelessness and ultimately work towards their recovery. Model of service is mobile, community based, client directed strength-based case management. Services can be provided anywhere in the community where the client may request support.

Access Information

Shelter referrals are identified by community homeless shelters and given to City of Ottawa for prioritization and referral to CMHA. Community Referrals go through CMHA's central intake function that manages referrals to CMHA as well as a number of community partners that provide population specific mental health case management services (Mental Health Community Support Services or MHCSS).
