Program Description

Assessment, consultation, pharmacological management and psychotherapy support for patient and family/caregivers. This program is designed to clinically assess and diagnose people with signs and symptoms of neurocognitive illness (e.g., dementia) and caregiver stress. Available in-office, at the hospital, or from program staff "on assessment" at an individual's home. Model of service is assessment, patient consultation, memory group for mild cognitive impairment, assessment of caregiver stress, support and education regarding the behavioural manifestations of dementia and delirium. Consulting with referring physician, family/caregivers for patients showing signs and symptoms of various forms of dementia. This service defines geriatrics as aged 55 and over. Model of service is consultation. Shared care.

Access Information

This is a community based service. Please do not send emails containing personal health information about self or family. Due to the prevalence of medical complications, physician involvement in referrals is essential. The clinic may assist clients in obtaining a physician, when access to physician rosters is available.
