Support Within Housing - Mental Health

Program Description

The program provides subsidized housing with supports to persons who have been homeless who are living with a mental illness. Services include care planning, life skills supports, advocacy, social recreational events, service coordination, and referrals. Services are provided by community mental health workers, peer support workers, program nurse and consulting psychiatrist. Tenants have access to Steps to Health, a lifestyle change and wellness program, and supportive employment opportunities.

Access Information

Self-referrals or referrals by professionals are accepted by telephone. Information requested is name, date of birth, information on homelessness and mental health history, and how to contact the person. When a unit is likely to become available, an intake interview is scheduled to provide detailed information on the program and its services and to collect more information on the potential tenant. There are times when referrals are not accepted and wait list is closed.
