Program Description

Intensive case management offers a supportive framework for individuals with serious mental illness to achieve greater independence and an improved quality of life. Our highly skilled case managers work in partnership with individuals to identify areas of strength and determine areas of need. Together, a support plan is prepared, addressing areas such as skills development, particularly as they relate to employment, issues related to primary healthcare, and access issues for housing and/or income; we assist clients to engage in their local community to reduce their experience of social isolation. In addition to intensive case management, individuals in South Simcoe who are homeless or at risk of becoming homeless, can access additional support to ensure housing stabilization. Model of service is recovery.

Access Information

Referrals are welcomed from all. Need to have a serious mental illness or be diagnosable.
