Program Description

The Community Support Program is a service which provides professional guidance and support in order to develop and maintain essential life skills needed for independent living while improving accessibility to community resources and reducing the incidence of hospital re-admission. The program objectives are to provide outreach services to individuals who may identify or appear to have a service need; to establish an individualized recovery action plan with a focus on client strengths and motivations; to advocate for the protection of rights and access to a seamless continuum of care; and to offer skills development and learning opportunities through psychosocial rehabilitation strategies. GAIN assessment tools are used for referral and screening process. Rational Emotive Behavioural Therapy is offered through this program.

Access Information

Referrals can be made by self, family, hospital, physician, psychiatrist, mental health or health professional. Referral forms may be accessed through the Cornwall office. Interim supports offered through groups and resource centre.
