Type of Service

Support Within Housing - Mental Health

Program Description

Cota Health's Scarborough Step By Step supportive housing program (Step By Step) provides rent subsidized accommodation and support to people living with mental health challenges in the Scarborough community. Each tenant occupies his/her own apartment or housing unit. The Step By Step program is operated in partnership with the Toronto Branch of the Canadian Mental Health Association (CMHA), Community Resource Connections of Toronto (CRCT), Scarborough General Hospital and the Ontario Shores Centre for Mental Health Sciences. Cota Health provides support to tenants through individual schedule visits and coordinated group activities. The staff work collaboratively with tenants, their families, the landlord and other clinicians, as appropriate to assist tenants to: develop an individual safety and housing support plan; maintain their housing stability and a health housing environment; acquire and/or maintain the skills necessary for daily living; participate in health promotion and community development activities such as tenant meetings, structured educational events and social activities; resolve situational problems/disputes; link to other desired community services/supports, as needed; achieve/maintain optimum health and wellness; and pursue personal recovery.
