Support Within Housing - Mental Health

Program Description

24 hour per day, transitional supportive housing for people with a serious mental illness. Program includes one-on-one life skills training, coordination of required services, conflict resolution and crisis prevention, discharge planning and referral to community resources.

Access Information

Contact program director for more information on the intake and admission process. Referral requires the completion of the intake forms showing history of hospitalizations, medication, and community living issues. Interviews and home visits are required to determine suitability.
