Program Description

The HOST Program provides mobile housing support services. We strive to provide services that will support individuals to recover from the effects of mental illness, homelessness, addictions and poverty. All services are offered using a recovery orientation of psychosocial rehabilitation practice. All tenants have access to an emergency on-call person after hours. All Community Mental Health Workers provide direct skills teaching, supportive counselling, liaison with other community resources, goal oriented individual functional and resource assessment, advocacy, social and recreational events and crisis prevention and intervention. All tenants also have access to our Community Mental Health Nurse, Psychiatrist, and Trustee. Average length of stay overall is 3.65 years.

Access Information

Referrals are through The Access Point. An intake assessment will be scheduled if the individual meets the criteria of living with a mental illness and are homeless or at risk of becoming homeless. Once the assessment is completed and the person is deemed to meet the criteria, they will be put on a waiting list if no vacancy is available. If a vacancy is available, the individual will be assigned a Community Mental Health Worker and shown the unit.
