Program Description

Our flexible program provides intensive case management services in northeast Toronto. This program is intended for individuals who have, or are at risk of having a serious mental illness and both need and want intensive supports to live satisfactorily and successfully in the community. Model of service is strengths model/client-directed, recovery using psycho-social rehabilitation principles. Average length of stay is 5.4 years.

Access Information

Professional referral to Case Management program is preferred but not mandatory. The selection of clients is based on the determined benefit of the program for the applicant to live satisfactorily and successfully in the community. All applications for Bayview Community Services Case Management services are processed through The Access Point. A potential client's application is only sent from The Access Point to Bayview Community Services when there is a vacancy available at Bayview Community Services. Once the application is received and reviewed by Bayview Community Services staff, the applicant/referring worker will be contacted. An initial meeting with a case manager will be set up to determine suitability of the program for the potential client. Afterwards, the applicant/referring worker will be contacted regarding the outcome of this meeting. Accepted applicants will be assigned to a case manager who will be in contact with the applicant and referring worker. Admission Criteria: Adults (16-65) who have a serious psychiatric disability and established ongoing professional follow-up. They must have an identified need for intensive community support services and willingness to accept support services from Bayview Community Services. Exclusion: those with ongoing histories of uncontrollable violent behaviour.
