Support Within Housing - Mental Health

Program Description

A medium support, co-ed group home in North Toronto with a capacity of 10 people. Residential counsellors are available weekdays in the afternoon and evening, and on afternoons on most weekends. Staff is available afterhours for emergencies. Residents must be able to live in a group setting with this amount of support. Residents share bedrooms. Model of service is client-directed, recovery using psychosocial rehabilitation principles.

Access Information

Professional referral to Support Within Housing program is preferred but not mandatory. The selection of clients is based on the determined benefit of the housing program for the applicant. Applications for Bayview House are processed through The Access Point which is responsible for managing the housing waitlist. Referral forms can be requested by calling Bayview Community Services office, accessing them on Bayview Community Services website or through The Access Point. Tours can be provided upon request. Admission criteria: adults 16 years of age and over who have a serious psychiatric disability, no recent street drug or alcohol abuse, financial status and professional follow-up established prior to admission, and no conviction under the criminal code. They also must manage and monitor own medication, need a long-term supportive environment in order to reach and maintain optimum level of functioning, be motivated to live in a cooperative setting and be involved with daily activity. They must follow non-negotiable house rules.
