Program Description

Outreach service through Case Management Services provides support and assistance to individuals 16 years of age and older who are living with a serious mental illness. Individuals can access the support and assistance of case managers for daily living skills, intensive support, coordination and referral to other services, while they strive to achieve and maintain a good quality of life in their home community.

Access Information

All services are professional, strictly confidential and are provided at no cost. Referrals for service are accepted from all avenues, including self, family member or friend, physician, other community services, etc. Service provision is voluntary and designed to involve and work directly with clients to achieve the best, most effective and efficient delivery of service.
