Program Description

The Getting Appropriate Personal and Professional Supports (GAPPS) program is a partnership among three organizations: St. Joseph's Care Group, Canadian Mental Health Association- Thunder Bay Branch, and NorWest Community Health Centre. The goal of GAPPS is to identify and respond to the unmet needs of a population of vulnerable persons with very serious, unstable and complex mental illness and addiction issues. The program will reach out to and engage with people who do not appropriately access health and social services. It will assist the target group in navigating health, mental health/ addictions and social services, including housing. The program will also facilitate access to clinical services and provide primary care in a less formal setting, in order to reduce reliance on emergency services for non-urgent health care needs. CMHA Thunder Bay provides the community outreach and engagement component of the program team, which also includes nurses, psychiatrists, case managers, etc. The program offers primarily short-term support.
