Case Management/Supportive Counselling & Services - Mental Health

Program Description

Client-centered case management promoting wellness for consumers of mental health services in Haldimand and Norfolk counties. These services generally meet three main goals of reducing hospital admissions, easing the duration while in hospital and assisting with a successful re-entry from the hospital back to their community. Support may include attending physician visits, medication monitoring, help finding housing, help connecting in the community, life and social skills support, help maintaining employment, court support, support with educational and volunteer goals, assistance in getting informal supports, and psychiatric support via OTN. Model of service is bio-psychosocial rehabilitation and recovery.

Access Information

A referral form is used by all mental health organizations in this service area. A person can be referred by self, family members, hospitals, doctors, social workers and other community organizations. Prospective client then attends an intake interview. A written doctor's diagnosis of a serious and persistent mental illness is required for the client to be placed on the wait list.
