Program Description

This program provides intensive case management to support people who are homeless or precariously housed. Eligibility includes: Individuals who are 16 and older living in York Region; Individuals who have a mental health concern, with our without substance use issues, or a developmental disability. Clients must be homeless, precariously housed, or living in social/supported housing. This program offers a variety of supports, as well as different levels of intensity. Low intensity Supports-support groups throughout York Region, as well as one-to-one support with referrals and systems navigation. Intensive case management program-one-to-one support, goal setting and assistance with housing stability. Expanded housing first program-Multi-disciplinary team that provides intensive case management for people who are homeless or precariously housed. High intensity community supports team-Client can receive up to six months of support from a multi-disciplinary team, which includes a psychiatrist.
