Program Description

Frontenac Assertive Community Treatment Team (ACTT) is a multidisciplinary team which provides outreach, individualized treatment, ongoing and continuing services, linkages, and includes a monitoring and evaluation component. Assertive community treatment is a client-centred, recovery-oriented mental health delivery model that has received substantial empirical support for facilitating community living, psychosocial rehabilitation and recovery for persons who have a serious mental illness, have severe symptoms and impairments, and have not benefited from traditional outpatient programs. Model of service is psychosocial rehabilitation.

Access Information

Referral forms must be completed and referrals can be made by a variety of sources including: family physicians, psychiatrists, family members, social workers or the individual requesting service. All referral forms must be signed by the individual who is requesting service. A medical is not required.
