Program Description

The Crisis/Outreach team provides assessment services to the Schedule 1 facility, offering initial screening and triage of referrals, crisis intervention and bio-psychosocial assessments. In situations where clients or families can be safely treated in the community, they are offered short-term intensive treatment. Risk management plans are developed if necessary. Treatment is time- limited and goal-oriented with special consideration given to the organization, linking and bridging to community support services. Clinical interventions are strategically designed to rapidly stabilize the client or family's individual circumstances, both from a mental illness perspective, as well as assisting with critical psychosocial issues related to successful community integration. Model of service is crisis intervention, solution-focused therapy (e.g., recovery, psychosocial, and telephone).

Access Information

The Mental Health Crisis Line of Leeds and Grenville provides a centralized access point for individuals experiencing a mental health problem. It is also a service that assists individuals in navigating the mental health system.
