Program Description

A recovery-based case management program offering services to Durham Region residents aged 16 and over who have serious, chronic mental health issues, want the services offered, and are inadequately linked to needed community supports. Through the development of a strong working partnership, our case managers help clients achieve and maintain community integration and enhanced quality-of-life. Helping Partnerships focuses on clients' abilities, strengths, and resources, and helps clients leverage these in pursuit of their recovery goals. The program offers a full range of services including; assessment, case planning, advocacy, referral, and skills training. The program will benefit people needing help meeting basic needs, such as housing, finances, education and employment, emotional support, and crisis management.

Access Information

Services are open to Durham Region residents aged 16 and over who have serious, chronic mental health issues, want the services offered, and are inadequately linked to needed community supports. Applications can be made by the individual requesting service or by a third-party referral (family member, psychiatrist, community support, etc.) Application forms can be obtained by calling our Central Intake or by downloading from our website, www.dmhs.ca. No toll-free line, but callers are welcome to call Central Intake collect.
