Program Description

Transitional Case Management offers services to people struggling with their mental health needs which have resulted in an impairment to functioning in daily living skills, managing finances, housing, health, social recreation and employment domains. Services are delivered within a framework of psychosocial rehabilitation and recovery and are driven by individual client needs and goals. Individuals are followed for 4-6 months with the intention of providing support in meeting individual's immediate needs and short term goals. Model of service is psychosocial rehabilitation and recovery, community based and voluntary.

Access Information

Individuals are not required to have a formalized mental health diagnosis to be eligible for Transitional Case Management support. The referrals to this program come from individuals in need seeking services at the Queens Ave or Huron St location, or by completing the on-line referral form found on the CMHA Middlesex website. Persons should be willing to meet regularly with a Transitional Case Manager to identify and work on their rehabilitation goals.
