Assertive Community Treatment Teams

Program Description

Our ACT team's services are committed to being flexible and responsive to the unique needs and desires of each client. The supports offered include: working with the client to find housing based on personal preferences; support in accessing basic necessities such as personal identification, food, clothing, health and income support; comprehensive bio-psychosocial assessment; collaborative development and implementation of an individualized recovery plan; outreach supports and the development of a trusting relationship between the client and the service provider team; assistance to achieve/maintain optimum health and wellness; and linkage to community resources (social, recreational and/or primary health services). In addition, direct support is offered in the areas of: education about the nature of mental illness; support and education to caregivers and/or the person's support network, as appropriate; development of self-management strategies; encouragement, emotional support and motivation; development of personal support networks; and development/maintenance of abilities relating to activities of daily living.

Access Information

Referrals are accepted through The Access Point. Eligibility criteria includes serious mental health challenge and homelessness, or at-risk of homelessness.
