Program Description

The ACT Team is a community-based, outpatient treatment program providing services to residents of Leeds, Grenville and South Lanark counties with a serious mental illness. Services are provided 365 days per year with evening and weekend services and 24-hour on-call availability; treatment in the client's environment; monitoring, assessment, and teaching about medications; housing assistance; vocational support; recreational support; life skills teaching; interagency liaison; and continuity of care with no limit on the length of time with the service. Model of service is psychosocial rehabilitation.

Access Information

Referrals are accepted from mental health care facilities, community providers, and family physicians. The team serve clients 16 years of age and older who are residents of Leeds, Grenville or South Lanark counties who are frequent users of mental health services, have significant functional impairment and continuous high service needs due to a serious mental illness. There is a joint referral form for a single access point for individuals diagnosed with a severe and persistent mental illness requiring supportive housing, ACT and/or case services for residents in Leeds and Grenville.
