Program Description

Community Day Services (CDS) offers accessible supports, with a goal of providing 'as needed' and 'just in time' service to prevent the need for re-admission and to reinforce client-centered goals for recovery. CDS consists of three program components: Clinics - Clozapine and Injection clinics, Dual Diagnosis, Metabolic Monitoring; Groups - Healthy Hints and Clozapine groups; and Individualized Recovery Supports - one to three sessions with a nurse, recreation therapist, dietician or spiritual care provider to discuss a specific goal and develop an action plan. More support providers will be added as the program expands. Open to anyone who has a support in community.
