Program Description

The Community Support Program (CSP) offers long term case management to youth aged 16-24 with a serious mental illness. Service includes individualized, flexible and comprehensive planning. Community Support Workers offer individual and group counselling, support, crisis intervention and linkage to other services and assist in providing a seamless transition to adult mental health services. Model of service is recovery.

Access Information

Referrals are accepted from hospitals, psychiatrists, community agencies, consumers and families. Referrals are accepted through The Access Point.
