Program Description

The South Simcoe Key Program provides intensive case management and housing support services to individuals in the South Simcoe area. Residents must have a serious mental illness, be 16 years of age or older, and homeless or at risk of becoming homeless. They also must agree to participate in the organization's intensive case management program. Model of service is recovery.

Access Information

Referrals are welcome from all sources. Individuals must have a serious mental illness, or be diagnosable, and have a history of homelessness or be at risk of becoming homeless. Initial intake process takes one to three weeks. Clients must be on Ontario Works or ODSP. Telephone interpretation service available for all inquiries in more than 170 languages.
