Program Description

Cota Health provides intensive 24/7 on-site support to clients of this program. The staff work collaboratively with clients in order to help them: develop an individual safety and short-term housing support plan; achieve temporary housing stability while seeking long-term housing options; acquire and/or maintain the skills necessary for daily living within the housing environment; access required services/supports from other sources (e.g. case management, primary care physician, identification, etc.); participate in health promotion and community development activities such as group meetings, structured educational events and social activities; resolve situational problems/disputes and pursue personal recovery.

Access Information

All beds are available to men and women living with a serious mental illness (which includes Dual Diagnosis and Concurrent Disorders) who are 16 years of age or older, are homeless or at risk of becoming homeless and who are assessed as being able to be safely supported within the program.
