Support Within Housing - Mental Health

Program Description

Support workers provide people with a serious mental illness, with individualized support that assists in meeting their dreams and goals to be successful in independent living. People reside in various settings in the community, including individual apartments and their own homes. Supports are offered in all aspects of life including, but not limited to, recovery, life skills, medication and health, recreation, community participation, healthy relationships, employment based on person-centred plans. The individual determines the length of stay.

Access Information

Primary diagnosis of a serious mental illness. Adults, over the age of 18, in receipt of Ontario Disability Support or other source of funding. Set up a tour through the Manager of Quality Assurance.
